Online Forms and Time sheets
Below are electronic forms and time sheets available for submission.
> Absence forms have been revised and can be submitted quicker and easier; failure to submit forms in a timely manner could result in incorrect reporting and dock of pay.
*Personal, sick, vacation, bereavement, etc. are all requested via one form
- Select the correct absence/personal leave request form from the drop down list. (It is important you select your correct building)
- Fill out and sign the form electronically, be sure to select the "click to sign" at the end in order to finalize the form.
- The form is then forwarded directly to your building principal/supervisor for approval/completion.
- Receive a finalized copy showing approval/denial and deducted day(s).
*If you submit a personal day request for a future date but end up working that day please contact your building principal or Angela McMahon at mcmahona@unit10.com in order to cancel/terminate the request.
> Time sheets may be printed or downloaded for your convenience.

> Facility Use Forms are now automated. Please select the form for the building you are wanting to use. (Select the arrow to scroll over and view these forms)
